Birth Certificates
The Seneca County General Health District can now provide certified copies of Birth Certificates for any person who was born in the State of Ohio. In the past you would have to obtain the certificate from the county of your birth. Statewide Issuance gives you the ability to obtain your Birth Certificate without the inconvenience of traveling to your birth county or applying by mail. Birth records prior to 1909 are kept with the Seneca County Probate Court and are not available through our office.
Certificate Fees
The fee for certified copies of birth or death certificates is $25 per copy. Methods of Payment include cash, check, credit card or money order. Credit Card purchases require an additional processing fee. Due to changes in State law, we can no longer provide uncertified copies of birth or death certificates.
To help reduce the potential spread of COVID19 SCGHD will be taking birth certificate & death certificate orders over the phone with a debit or credit card. To place an order over the phone, please call 419-447-3691 Ext. 300 or 301.
The Vitals Statistics Office (Birth & Death Records) will be closed from 12:00 - 1:00 for lunch. We are sorry for any inconvenience.

How to Order
You may order copies of certificates by mail, in person or online with a credit card (additional processing fee applies).
In Person: Visit the Vital Statistics Office at the Seneca County General Health District. In most cases your certificate will be processed and issued in about 15 minutes.
By Mail: Mail the completed Birth Certificate Request Form along with payment for the certificate and a self-addressed stamped envelope to:

Upon receipt of your request, your certificate will be mailed back to you within 2 business days. Please allow 7-10 business days for the return mail. When requesting by mail please include the following information: Name on record, Date of birth, County of birth, Mother’s maiden name, Father’s name.
Online: For your convenience, you can process online requests through VitalChek Network, Inc. – an independent company that the Seneca County General Health District has arranged with to provide you this service.
VitalChek may be reached through their web site. An additional fee of $7.00 (plus shipping) is charged by VitalChek for this service. All major credit cards are accepted, including American Express, Discover, MasterCard or Visa.
